Documentary Requirements for Skilled Workers

Letter of Intent addressed to the Labor Attaché on the intention to apply for accreditation and/ or job order. (click here to download)

Initial/ Renewal

1. Recruitment Agreement (RA) or Special Power of Attorney (SPA).
   (click here to download template and must be attested).
   Note: If the RA or SPA was signed digitally, an audit trail certificate is required to be attached to these documents click  here for advisory)

To submit as attachments of the RA or SPA the following:

• • Clear photocopy of any valid ID of the signatories to the RA/SPA;
  • Commercial license of Principal/ QRA with English translation
  • Extract of Commercial Registration with English translation
  • Computer ID with English translation
  • For health and medical sector including nursing agencies, Ministry of Health license with English translation (Templated with attestation)
  • Photocopy of DMW valid license of the PRA.

2. Job Order (click here to download template and must be attested)
   To submit as attachment the following:

• • Visa Approval
  • Undertaking on Visa Usage (if the actual position is different from the visa position)

3. Master Employment Contract (MEC)
   (click here to download template and must be attested).
4. MWO Qatar Certificate of No Pending Case
5. Affidavit of Undertaking re: Undertaking by the Principal/QRA and PRA to monitor the employment of overseas Filipino workers and to submit a report of significant incidents relative thereto, DMW Advisory No. 24-A series of 2024; POEA Advisory 21, series of 2018 and letter of guarantee on the health insurance (Click here to download template and must be attested).
   Note: If the Undertaking was signed digitally, an audit trail certificate is required to be attached to these documents. (click here for the advisory)
6. Comprehensive and Actionable Contingency Plan
7. Clear copy of the blue plate, location map and photos of office and accommodation (2 pages only)
8. Accomplished Accreditation Form

Additional requirements for companies/ employers with QRA

10. Service Agreement, attach the following:

• • Manpower requirement/demand from its client- company/ employer (Templated with attestation)
  • Commercial license of Principal/ QRA with English translation
  • Extract of Commercial Registration with English translation
  • Computer ID with English translation
  • For health and medical sector including nursing agencies, Ministry of Health license with English translation
  • MOL license of QRA w/ English translation.

Additional requirements for Multiple Accreditation

12. Undertaking to comply with all its obligations to the other PRAs to which it is currently accredited

13. MWO Qatar Clerance/ Certificate on No Pending Case;
14. Certificate of No Pending Case or certificate from the PRA that no case filed against the principal/ employer.
