Documentary Requirements for Household Service Workers

Initial/ Renewal

1. Recruitment Agreement (RA) or Special Power of Attorney (SPA). (click here to download template and must be attested).
   Note: If the RA or SPA was signed digitally, an audit trail certificate is required to be attached to these documents. (Link to the Advisory and sample)

To submit as attachments the following:

• • Clear photocopy of any valid ID of the signatories to the RA/SPA;
  • Commercial license of QRA with English translation
  • Extract of Commercial Registration with English translation
  • Computer ID with English translation
  • Photocopy of DMW valid license of the PRA.

2. Job Order

   To submit as attachments the following:

• • MOL license of QRA w/ English translation; and
  • Deployment Report, if applicable.

3. Standard Employment Contract (SEC) signed on all pages by the authorized representative of the employment agency.

4. MWO Qatar Certificate of No Pending Case
5. Certification
   (click here to download) and Proof of Employment or Undertaking to Hire Filipino Welfare Desk Officer/ Counsellor
6. Affidavit of Undertaking re: Undertaking by the Principal/QRA and PRA to monitor the employment of overseas Filipino workers and to submit a report of significant incidents relative thereto, DMW Advisory No. 24-A series of 2024; POEA Advisory 21, series of 2018 and letter of guarantee on the health insurance.

7. Comprehensive and Actionable Contingency Plan
8. Clear copy of the blue plate, location map and photos of office and accommodation (2 pages only)
9. Accomplished Accreditation Form

Additional requirements for or Second Accreditation

10. DMW ICT Deployment Report indicating at least 100 newly-hired HSWs
11. Proof of employment of at least one (1) Filipino Welfare Desk Officer;
12. Undertaking to comply with all its obligations to the other PRAs to which it is currently accredited
    (refer to Number 7.Affidavit of Undertaking Dual Accreditation or click here to download template and must be attested)

Additional requirements for Multiple Accreditation (may be considered only after one year from the last accreditation)

13. DMW ICT Deployment Report indicating at least 200, 300, or 400 newly-hired HSWs
14. Certificate of no pending case from the Adjudication Office of the DMW
15. MWO Qatar Clearance;
16. Certificate of no Objection from the PRAs it is currently accredited
17. Undertaking to comply with all its obligations to the other PRAs to which it is currently accredited

22. Letter of no objection issued by the previous PRA
